Please fill out all of the patient forms completely with BLACK ink only and REMEMBER to BRING THEM WITH YOU on the day of your appointment. DO NOT MAIL THESE FORMS BACK TO THE OFFICE.

 

Please plan to arrive 30 minutes prior to your appointment in order to find parking and to make sure we have time to input your information.

 

If you would like your insurance billed for this visit, YOU MUST BRING YOUR INSURANCE CARD WITH YOU and have it ready to present at the time of check in. If this is a problem, please call your insurance company PRIOR to your visit with us to get some form of proof of coverage.

 

In order to keep our obstetrical patients from being exposed to communicable diseases, the doctors ask that you not bring sick children into the office.
